Plano Family YMCA Registration Checklist

Your camper's registration is complete once the following are fulfilled:

____  Paid in full two weeks prior to camp session start date

____  Enrollment form has been completed and received

___   Camp Session Selection sheet is completed and received

____  A current copy of your child's immunization record is on file
        *You must submit a new copy even if your child has participated in 
         AfterSchool, Pre-school or previous camp seasons.

____  A current copy of your child's vision and hearing test is on file  
         *You must submit a new copy even if your child has participated in 
         AfterSchool, Pre-school or previous camp seasons.
